As of today, it seems like sfw content containing minor characters is no longer hidden when browsing at a higher level than PG-13. Citing from this pull request there seems to be yet another policy change:

Hide minor-flagged content only when it is also mature (nsfwLevel > PG-13). SFW minor content (PG / PG-13) is visible at all browsing levels. .red retains full content-level control including SFW-only."

Reading more of that pull, it sounds like the previous policy of hiding minor content was due to the possibility that images with both minor flag and R+ content could be surfacing to users:

"Because the server no longer serves minor+mature to non-mods, the widened client gate has no mature-minor payload to reveal even in the worst case; for moderators disableMinor is off anyway (they may see minor content)."

This problem appears to be solved now and probably was the reason to hide minor content even after the .red switch.

Update (14th of July 18:30 UTC): Visibility of minor content has been reverted to PG and PG-13 browsing levels as before. Seems like this was a mistake (done by Claude?):
"Provenance of the "SFW minor visible at all levels" line in #3095's body: it came from a mid-work clarification, phrased as "hide minor content only when nsfwLevel > PG-13, show SFW minor content at all levels?" — answered yes. That phrasing never distinguishes SFW content from an SFW viewer, and the entire bug lives in that gap. Today's reaffirmation supersedes it."
